General Inquiry

Effective immediately.  In order to communicate via email you must sign an email consent form.  Signed consent can be mailed/faxed or dropped off at reception.  Print off a consent form from our website. 

Please note that you will not receive an email response from our office unless we have a signed consent on file.


Urgent requests are not to be sent through our website.

Contact our office by phone if you do not receive a response within 24 hours.

If you require a prescription refill you can contact your pharmacy and request a refill fax to be sent to your physician.
