We appreciate your patience during this transition period as I take on Dr. Nemtean’s practice and settle into a new office. At Summerville Family Health Team, we recognize that changing physicians may be stressful. We do endeavor to make the process as seamless as possible, especially when a long-time physician such as Dr. Nemtean retires. I feel privileged to take over your care after his departure.
The Ministry of Health requires that a new enrolment form be signed by each patient to remain with the practice after a change in physicians. I’m hoping you can address this easy administrative task as soon as possible, thus ensuring you continue to have access to all the programs and services that our Summerville interdisciplinary team provides. Failure to return the signed form by June 30, 2026, may result in your status as a patient in the practice being discontinued.

You may return the signed form using any one of the following methods:
Drop off completed forms at our clinic’s reception desk (blank form are also available for you to fill at the clinic)

Unfortunately, there is currently a longer than typical wait time for routine appointments to transition care. Some appointments are held on a first come, first serve, basis for urgent concerns only and for urgent test results that come through daily. If I am unable to accommodate you with a timely appointment, there may be appointments available with our Nurse Practitioner, which you can access through MEDEO.
Alternatively, you can always visit one of the Summerville After-Hours Clinics that run on weekday evenings, Saturdays and Sundays, for urgent needs only. After-hours clinics are available to all Summerville patients and are not site specific. Please check the Summerville website for the schedule and location at: After-Hours Care. There is also the Mississauga Paediatric Care Clinic, managed by Summerville, that is available for children and youth aged 6 months to 17 years old who do not have a family physician, or their family physician does not have an after-hours clinic to assist with certain non-urgent care needs. If you decide, you do not wish to remain with the Apple Hills practice; I will need to receive written authorization from you to transfer your records to a location of your choice. I will endeavor to do those chart transfers promptly. (Chart transfer fee is applicable).
